Student Vaccine Portal is Live

Vax LMU, the university’s new vaccination confirmation portal is now live for students. In preparation for LMU’s phased return to campus, students are required to provide the university with confirmation showing they have been vaccinated. Vaccination confirmation will be collected by the Dean of Students and the Student Vaccination Verification Team and will be maintained with the strictest privacy pursuant to federal and state requirements, including the Federal Educational Rights and Privacy Act (FERPA). The system, which is only accessible by necessary authorized persons, is easy to use and can be accessed via computer or mobile phone. Please wait until you have received your final dose before submitting your vaccination record. Scanned or photographed images of vaccination record cards will be accepted. Please ensure your image is clear and legible, and that all vaccination dates are provided.

All other vaccinations, including the required measles, mumps and rubella vaccine, are to be recorded and documented on myhealth.lmu.edu.

Medical or Religious Exemptions

A form will be shared with students in early June to request a medical or religious exemption to the vaccine requirement. Students who are granted an exemption may be required to use additional personal protective equipment or take other actions, such as participating in ongoing testing for the health and safety of the university community.
