Patient Rights & Responsibilities
Student Health Services wants you to receive quality care. As a patient, it is important to realize that you have both rights and responsibilities. This summary of patients’ rights and responsibilities describe how you can participate in receiving quality care.
You have the RIGHT:
- to be treated with respect, consideration, dignity and privacy
- to confidential treatment of all personal disclosures and records, and except when required by law, to have the opportunity to approve or refuse their release
- to complete and accurate information about your health-related concerns including diagnosis, treatment plan and prognosis. When it is medically inadvisable to give such information to you, the information will be provided to a person designated by you or to a legally authorized person
- to be given the opportunity to participate in decisions involving your health care, except when such participation is contraindicated for medical reasons
- to consent to, or refuse, any care or treatment
- to select and/or change your health care provider
- to review your medical records with a clinician
- to information about services and any related costs at the Student Health Service
- to express suggestions, complaints and grievances, including those required by state and federal regulations
- to have an advance directive (living will and/or durable power of attorney for health care decisions)
- to obtain information regarding an advance directive
- to have your advance directive (if you have one) included in your medical record
- to have your advance directive followed to the extent that is medically appropriate and lawful
You also have the RESPONSIBILITY:
- for your own health and to seek medical attention promptly
- to be honest and direct with the Student Health staff and to inform your health care provider of:
- complete and accurate information about your health and medical history
- medications being taken that have been prescribed by health care providers, as well as, OTC products and dietary supplements
- allergies or sensitivities
- objections to the proposed treatment plan
- to inform the staff if you have an advance directive
- to give SHS a copy of your written advance directive (if you have one)
- to ask about anything you do not understand
- to follow the health care providers advice and medical instructions
- for your actions if treatment is refused or instructions are not followed
- to communicate any significant changes in symptoms, side effects or failure to improve
- to respect the policies and guidelines of Student Health Services
- to be respectful of and courteous to the Student Health Services staff
- to keep appointments and be on time or to cancel in advance
- to provide useful feedback about services and policies
