 Q. How do I purchase the school insurance plan?

A. Loyola Marymount University endorses a student Injury and Sickness Insurance Plan underwritten by United Healthcare Insurance Company (UHCSR). Please contact the LMU Student Account Office at (310) 338-2711 to purchase the insurance policy.

Q. Is it mandatory that I purchase the Sickness insurance plan offered by LMU?

A. It is mandatory to show proof of insurance coverage at the time of your payment for classes if you are an undergraduate, graduate or international student enrolled in 7 or more semester hours.  You have an option to purchase coverage of your own choice and provide proof of coverage on your calculation of charges worksheet or purchase a policy through LMU on your calculation of charges worksheet. All foreign-born students must purchase this plan.

Q. What about the Injury plan?

A. All LMU undergraduate, graduate student and international students enrolled in 7 or more semester hours are required to purchase this plan. The cost of this plan cannot be waived and is included in your University fees.

Q. What are the costs of these plans?

A. The Injury Plan cost is $201. The  Injury and Sickness Plan cost is $874 per year.

Q. Please explain the difference between the injury policy and the sickness policy.

A. The Injury plan covers injuries only. The Sickness plan covers treatment for the care of illness.These plans have a number of exclusions. Please see the Plan Brochure for full details.

Q. Do I have to purchase LMU's Insurance Plan to be eligible for services at the Health Center?

A. No. Health services are available to all students enrolled in 6 or more units.

Q. Will the Student Health Center bill the insurance company for services I receive?

A. No. Every LMU student has a student account. The Student Health Center will bill your student account for charges incurred during your visit (such as medicine, lab work, x-rays or medical supplies). At the end of the month you will be billed by the Controller’s office. The billing statement will indicate “Health Service Fees,” with the amount owed. No details of the charges are divulged on your bill to Student Accounts for Patient Confidentiality purposes. 

Q. Is there a deductible?

A. Yes. The deductible is $200 for in-network care per year ($400 for out-of-network care per year).

Q. Does this insurance cover me when I go home?

A. Yes, the coverage is 24 hours a day, 7 days a week regardless of where you are.

Q. I want to submit my charges to my insurance company. How can I get a copy of my billing?

A. You will need to fill out a medical release form and provide proper identification (One-Card or Driver’s License). You may obtain a medical release form by visiting our office or going online to www.lmu.edu/health. Click on “downloadable forms” and then “click” on medical release forms. Print this form, complete the necessary information and mail or fax it along with a copy of your Driver’s License to our office at (310) 338-4417. It will take 7 to 15 business days to process your request. If you recently had lab work and wanted the request to include these charges, it may take up to forty days due to the lab's billing cycle.

Q. Can my parents access this information?

A. If you are a minor (under 18 years old) your parents have a right to your medical records. If you are 18 years or older, no person can access your records without your written permission.

Q. How do I submit a claim?

A. Directions on how to submit a claim can be found on our website, www.lmu.edu/health. Just click on “Health Insurance Billing” and you will find directions on how to submit a claim.

Q. I didn’t receive my insurance card. How can I get a card?

A. There are three ways to obtain an insurance card. The insurance company mails a card to your permanent address early in the fall semester. Second, an insurance identification card is available inside of the insurance brochure. You can pick-up a brochure at the Student Health Center or call our office (310 338-2881) and we will mail you a brochure. Last, you can go online and request a copy at www.uhcsr.com  You will need to register (using your name and UHCSR identification number). If you do not have your UHCSR identification number, please call UHCSR at (877) 848-7248 to retrieve this number.

Q. Can I go to my own doctor or do I need a referral?

A. A referral is required in most cases. However, a referral is not required if 1) you are more than 50 miles from the campus. 2) it is a medical emergency. 3) the Student Health Center is closed. 4) service is rendered at another facility during LMU break or vacation periods. 5) the service is for psychotherapy or maternity care. 6) injuries sustained while participating in an NCAA sanctioned sport. 7) you are no longer eligible to receive services at Student Health Services due to a change in your student status.

Q. Do I need a referral for Emergency Room follow-ups?

A. Yes. Your insurance coverage requires all follow-up visits from the emergency room be done at the Student Health Center unless you meet the criteria listed above.

Q. How can I get the most benefit out of this insurance plan?

A. First and foremost, come to the Student Health Center. Please call early (before 9:00 AM) if you need to schedule a same day appointment. If you are away from the campus and want to visit a doctor, stay within the PPO (Preferred Provider) network. You can find a PPO provider online by logging into www.uhcsr.com. Follow the prompts. You can save money by staying in the network and receiving a discount.
