- Application for Degree
Every graduating student must submit this form in the term in which the degree is to be granted. This application must be filed again if a degree is not awarded in the term for which you applied.
- Application for Summer Registration
Non-LMU students who wish to enroll in undergraduate courses may use this form for Summer registration.
- Authorization to Release Student Records
Once a student enrolls at LMU, the student controls access to his/her record. Please consult the section on FERPA so you understand what you are releasing and why.
- CAPP Adjustments
CAPP is the official degree audit for all LMU students. Please click if you wish more information on CAPP. The adjustment form allows for course substitutions, rule waiver, and other adjustments.
- Change of Name or Number
Use this form to change how your name appears on LMU records. Documentation of the new name will need to be attached to the form.
- Change of Program or Core
Use this form when adding, changing, or dropping a major or minor program. This also will allow for the change of concentration within degree programs using them.

- Diploma Request after Account Hold Use this form to request diploma after financial obligations have been met.
- Duplicate Diploma
LMU alumni may order a duplicate of their original diploma at any time
- General Petition
This form is to be used for students petitioning for an exception to University academic policy. The second half of the form is to be used for: time conflicts, unit overloads, CR/NC grading, course audit, incomplete extensions, course challenge, and exceptions to commencement guidelines.
- Independent Studies/Tutorial
If an instructor is willing to offer you either an Independent Studies or Tutorial course, use this form to facilitate the request to have the course approved. Please ensure you understand the requirements of these courses.
- Leave of Absence/Withdrawal
A Leave of Absence is a temporary break in a student's enrollment in all college-level courses whether at LMU or any other institution. A withdrawal from the University is the termination of student status at Loyola Marymount University.
- Request for Non-disclosure of Directory Information
Once a student enrolls at LMU and/or turns 18, the student controls access to his/her record. Please consult the section on FERPA so you understand what you are withholding and why.
- Reset PROWL Account
Use this to reset your password or if you don’t remember your account information or if your account has been disabled. PROWL accounts are available for all students who attended LMU after August 1, 1988.

- Transfer Course Approval (TCA)
Students wishing to take courses elsewhere and have the credit count towards the LMU degree must have a pre-approved TCA form. Courses which have NOT been previously approved may not transfer to LMU. Consult Transfer Credit in the Bulletin.
- Transfer Course Approval for Study Abroad
No form is needed to participate in an LMU-taught Study Abroad Program. If you wish to participate in an Exchange Program, you must submit the planned courses on the TCA form and obtain the signatures identified on the form prior to participation in the Program.
- VA Semester Benefits Entitlement (SBE)
Students receiving VA benefits must complete a Semester Benefits Entitlement (SBE) form each semester and submit it to the VA Certifying Official in the Office of the Registrar. The Office of the Registrar will not certify Benefits Entitlement for veteran students without a current SBE on file.
