Current Students
All students registered with the DSS Office intending to receive services during a given semester, must complete a Student Contract at the beginning of each term. Please Download and complete the Student Contract the sign up for a time to discuss your schedule with the DSS staff.
Student Contract (Acrobat) / (MS Word)Reasonable Accommodation Form (Acrobat) / (MS Word)This form must be completed for each class you intend to use your accommodations. Only enter your name, course and semester, DSS will complete the rest when you meet. DSS must sign off on your accommodations before you may present this form to your professor.Request for Proctoring Services Form (Acrobat) / (MS Word)REMEMBER, this form must be completed by both you and your professor and returned to DSS 7 days before your exams and 14 days before your finals.
Prospective Students
All students who wish to apply for accommodations with DSS must complete a Request for Services form. Other documentation will be required. It is best to contact the DSS Office to find out what additional forms are needed since documentation varies depending on each student and their disability. This form is must be filled out by the student who is requesting services.
Release of Information (Acrobat) / (MS Word)
This form is required if the student wishes for DSS to discuss their file with a third party. They are required to provide DSS with the name of who we may disclose information to as well as why type of information to disclose.
