CBL Evaluation Forms

SEMESTER-END
COMMUNITY BASED LEARNING EVALUATIONS

At the end of the semester, all students in CBL courses are required to submit two evaluation forms in order to receive credit for the service they have performed in conjunction with the class.  

(Each of the evaluation forms can be downloaded below by clicking on their names.)

1.  THE STUDENT EVALUATION FORM:
This form is to be filled out entirely by the student, and offers the opportunity for the student to provide important feedback on the quality of the service placement and the relationship of the service to coursework.  At the discretion of the course instructor, the student may turn this form in during class time or directly to Lorena Chavez in the Center for Service & Action (Malone 360).

2.  THE SUPERVISOR EVALUATION FORM:
Students should fill out the top portion of this form, and then give it to their direct supervisor at the volunteer service placement.  The supervisor may provide feedback on the number of hours and the quality of the student participation.  If as student is to receive credit for the service experience, the supervisor must sign and return this form. The student can offer to bring the form back to the CSA directly, or the supervisor may mail or fax the form to Lorena Chavez in the Center for Service & Action (instructions at the bottom of the form).
