SUBJECT: Spring 2008 Course Evaluation Forms

The course evaluation forms for spring 2008 are scheduled to be delivered to each department beginning Monday, April 14th. This will allow approximately two and a half weeks for the administration of the evaluations. The completed packets must be returned to all drop boxes by Monday, May 5th.

The procedure will be the same as last semester. Each packet will include a sheet of instructions that gives detailed directions to the students on the proper completion of the forms. A few extra forms will be included in each packet in case a student makes a mistake on the form in pen and wants to get another form.

The evaluation forms will be pre-encoded with course-specific information, so please be sure to take the correct packet to each class. Evaluation packets will not be prepared for courses with an enrollment of less than five students or for courses providing individual instruction.

Completed forms for each class should be returned to their original envelope and deposited by a student in one of the two locations below:

Von der Ahe Library - Main floor (during operating hours)

University Hall - 4th floor walkway (opposite the entrance

to my office – Suite 4820)
