How to Request Medical Records
Q. How can I get my medical records?
A. You must fill out a medical release form and provide valid picture identification (Driver’s license).
Q. Where can I get a medical release form?
A. You can retrieve a medical release form at either:
The Student Health Center, located at the Fritz Burns Athletic Complex, 1st Floor North or Online at www.lmu.edu/health
- Click on “Forms”
- Click on “Medical Release Forms” and print the form.
- Complete each section carefully and remember to sign and date.
Q. Can I get my medical billing information by using this form?
A. Yes. Make sure you check the appropriate box for “medical bills for insurance reimbursement,” and provide correct range of dates on the medical release form.|
Q. After I complete the form, what is the next step?
A. Present the signed form and valid identification to the Student Health Center OR
B. Mail the signed form and valid Identification to the Student Health Center at:

Q. Does it cost anything?
A. Your first copy is free of charge, the second and additional copies are
$10.00 each.
Q. How long does it take to process?
A. The normal processing time takes 7 to 15 business days. If you recently had labs taken, the process time may take up to 30 days to process, due to the billing cycle.
