MEDICAL: Blue Cross PPO; Blue Cross HMO; Kaiser HMO

The University offers a choice of three medical plans, consisting of one PPO plan and two health maintenance organization (HMO) plans. The PPO plan reimburses a percentage of covered charges after the deductible is satisfied. Both HMO plans offer comprehensive coverage by designated providers. Participants electing single coverage will pay a flat monthly co-pay designated for each of four salary tiers; participants electing dual or family coverage will pay a percentage of the monthly premium designated for each of the four salary tiers. DENTAL AND VISION: CIGNA PPO/Indemnity; CIGNA Dental Care; Vision Service Plan

The University offers a choice of dental plans, either an indemnity (PPO) plan or a prepaid plan similar to a health maintenance organization. The PPO plan reimburses a percentage of covered charges after the deductible is satisfied. The prepaid plan provides comprehensive dental care through designated providers. The vision plan also provides comprehensive ophthalmic care and materials through a provider network. The University will pay the full cost of single coverage and a portion of dependent coverage in the PPO plan and will pay the full cost of all coverage categories in the pre-paid plan. LIFE: MetLife

The University offers a choice of six levels of term life insurance for each faculty/staff member. LMU pays the premium for up to $50,000 coverage per faculty/staff member. Any premium for additional coverage will be deducted from the paycheck on a pretax basis. Term life insurance is also available for spouse/domestic partner and dependent children. The cost for spouse/domestic partner and dependent children insurance will be deducted from the paycheck on an after-tax basis.

ACCIDENTAL DEATH & DISMEMBERMENT: Combined Insurance of America (formerly Transamerica)

The University offers a choice of eight levels of coverage for each faculty/staff member, or eight levels of family coverage. LMU pays the premium for up to $50,000 coverage per faculty/staff member. Any premium for additional coverage will be deducted from the paycheck on a pretax basis.


LONG TERM DISABILITY: Prudential Insurance Company of America

The long term disability plan replaces a portion of the faculty/staff member's income in the event of a covered long term disability. The plan also provides basic contributions to the faculty/staff member's accounts in the University's Retirement Plan while s/he is receiving long term disability benefits. The University offers a choice of three (3) coverage levels. LMU pays the cost up to a 60% benefit level. Any premium for additional coverage will be deducted from the paycheck on a pretax basis.

LONG TERM CARE: CNA Insurance Company

The University offers four long term care options to each faculty/staff member and spouse/domestic partner. The plan will provide benefits for nursing home, home health care or adult day care as a result of injury, illness or aging. The cost of coverage will be deducted from the paycheck on an after-tax basis. Parents, grandparents and parents/grandparents-in-law of the faculty/staff member are eligible to apply and will be billed directly by the insurance carrier.
